LETTER TOTHE EDITOR Intravenous thrombolysis for ischaemic strokes: a call for reappraisal

چکیده

Sir, Balami et al. (2013) state that intravenous tissue plasminogen activator thrombolysis is beneficial for the treatment of acute ischaemic stroke and that strokes are now a treatable medical emergency. A critical analysis of the data, however, raises doubts about the expansive claims of efficacy made by them and others (Wardlaw et al., 2012). The aim of tissue plasminogen activator treatment is lysis of the thrombus and recanalization of occluded cerebral arteries, to promptly restore blood flow. To prove therapeutic efficacy of the administered thrombolytic agent, angiographic demonstration of arterial recanalization is critically important as it is the only way, in patients, to conclusively demonstrate that clot lysis has occurred.

Reply: Intravenous thrombolysis for ischaemic strokes: a call for reappraisal.

Sir, Many thanks for giving us the opportunity to reply to the letter in reference to our review ‘The exact science of stroke thrombolysis and the quiet art of patient selection’ (Balami et al., 2013). Aside from well documented limitations, recombinant tissue plasminogen activator (rt-PA, alteplase) remains the only evidence-based treatment in acute ischaemic stroke (Balami et al., 2013) with ...

Letter by Radecki regarding article, "safety of thrombolysis in stroke mimics: results from a multicenter cohort study".

BACKGROUND AND PURPOSE Intravenous thrombolysis for acute ischemic stroke is beneficial within 4.5 hours of symptom onset, but the effect rapidly decreases over time, necessitating quick diagnostic in-hospital work-up. Initial time strain occasionally results in treatment of patients with an alternate diagnosis (stroke mimics). We investigated whether intravenous thrombolysis is safe in these p...

A case of acute myocardial infarction 24 hours after acute ischaemic stroke.

Dear Editor, It is well known that thrombolysis for acute myocardial infarction (AMI) may lead to disintegration of pre-existing intracardiac thrombi with subsequent cerebral embolisation. It appears plausible that the same mechanism could apply to patients with cardiac or arterial thrombi who are undergoing intravenous thrombolysis for acute ischaemic stroke. AMI immediately following tissue p...

ASTRAL-R score predicts non-recanalisation after intravenous thrombolysis in acute ischaemic stroke.

Intravenous thrombolysis (IVT) as treatment in acute ischaemic strokes may be insufficient to achieve recanalisation in certain patients. Predicting probability of non-recanalisation after IVT may have the potential to influence patient selection to more aggressive management strategies. We aimed at deriving and internally validating a predictive score for post-thrombolytic non-recanalisation, ...

Reasons and evolution of non-thrombolysis in acute ischaemic stroke

INTRODUCTION Despite increasing evidence of its efficacy in advanced age or in mild or severe strokes, intravenous thrombolysis remains underused for acute ischaemic stroke (AIS). Our aim was to obtain an updated view of reasons for non-thrombolysis and to identify its changing patterns over time. METHODS This is a retrospective study of prospectively collected data from the Acute Stroke Regi...
